Sarah meets with Lincoln to find out about Michael. Lincoln says that Michael will get the surgery he needs from the Company's doctors. Sarah says she doesn't understand. The General walks in and tells Sarah not to worry. She questions his motives. He tells her that Michael will be treated as a guest in his house. Lincoln tells Sarah it will be alright.

Sarah goes to Michael while he is prepared for surgery. The doctor explains that Michael will be awake during the procedure. Michael asks what hospital he's in. Sarah tells him that he is in good hands. He asks for Lincoln and Sarah tells him that he is upstairs.

The General and Lincoln argue over the terms of their deal. Lincoln is then brought to T-Bag. Lincoln asks where Gretchen and Self are. T-Bag asks for compensation for his answer. Lincoln punches T-Bag, then starts to pull out his teeth with a pair of pliers. T-Bag screams out that they went to the pier at 3:00 to meet with a buyer for Scylla.

Sarah asks the doctors how many surgeries they have done and they assure her that Michael is in good hands. The surgeon then begins the operation by drilling into Michael's skull. The doctor tells Sarah to expect Michael to start having his life pass before his eyes as they press on different areas of his brain.

Lincoln meets with Sucre and tells him that the Company is operating on Michael in exchange for Scylla. Lincoln asks Sucre to help him find Gretchen and Self. Sucre is hesitant, but Lincoln asks him for just one more day.

Gretchen and Self wait on the pier for the buyer. Gretchen tells Self to let her handle the buyer, but he says no. Gretchen tries to seduce Self, but he tells her he is not interested and she shouldn't embarrass herself.

Mahone is driven away by Felicia and Wheeler. He asks to use the bathroom and cites the legal code that says he must be afforded the opportunity. Wheeler tells Mahone he'll let him use the bathroom, but reminds him of the code that allows him to shoot Mahone in the back of the head if he feels he is running away.

Michael sees himself in his prison cell. He walks over to the sink and Charles says hello. He tells Michael the answer to the question of whether or not he is dead is not. Michael asks why he's there and Charles says they all need the answer to that question.

Wheeler walks Mahone into the bathroom. Felicia checks the bathroom out and says it's all clear. Mahone is given 15 seconds. The agents wait outside, then wak in after 15 seconds. Mahone walks out of the stall and tells Felicia they should have trusted him like he trusted her.

The surgeon tells Michael that they will be isolating the tumor. He asks Michael to recite the alphabet while he inserts metal probes into his brain. Michael then sees himself back in the cell. Michael apologizes to Charles. Michael says it's hard to believe they lost and it was all for nothing. Charles says it doesn't have to be for nothing. Michael looks around and sees the most wanted posters featuring him and his friends.

Sucre and Lincoln drive to the pier. Gretchen and Self are still waiting for the buyer. Gretchen gets a call from her sister. Self tells her not to answer it, but Gretchen ignores him. Rita tells Gretchen they're free. She explains what happened and Gretchen tells Self that the Company took T-Bag. Gretchen asks about Emily and Rita says she needs to stay away from them. Self tells Gretchen not to worry about T-Bag. She says that he knew their location so the Company knows now too.

Sucre and Lincoln arrive at the pier. They look around, but see that they just missed Gretchen and Self. They see a bloody trail. Lincoln calls the General while Sucre explores the pier. Sucre brings the broken cell phones to Lincoln and they realize that Gretchen and Self know they're onto them. A man on the pier makes a call and says somebody is there, but not with Scylla.

Back in the car, Mahone asks why they are turning on him. Felicia says that they are just upholding the law. Wheeler sees Mahone looking out the window and asks what he's looking at. Mahone then kicks out the window, opens the door, and escapes into the woods.

The General meets with Sarah who is watching Michael's operation. He asks her what she sees in Michael and asks if she just likes the bad boy in him. She tells him that she is looking for the man responsible for her father's murder. The General tells Sarah not to worry about Michael because he is being taken care of. Sarah says she doesn't recognize the equipment being used on Michael. The General says that the equipment has been used before and the patient lived a long and happy life.
