A man falls during the filming of a commercial for a fitness camp. The man says he has cramps, but the instructor pushes him to keep going. After helping the man reach the top of the bleachers, the woman glazes over, then falls and rolls down the bleacher risers.
Cuddy grabs the elevator and rides with House. She tells House about the fitness trainer named Emmy who suddenly couldn't breathe, then fell and broke her ankle. House and Cuddy walk down the hall and Cuddy tells House she is taking his office since he was responsible for destroying hers.
House meets with his team and asks Taub where Thirteen is. He says she is getting treatments in the clinical trial. Taub thinks their patient could be suffering from steroid complications, but Kutner says that she is all about natural fitness. Cuddy listens in on the discussion and butts in at every suggestion. Cuddy then suggests that the patient could have suffered from exercise induced asthma.
Thirteen is in the clinic and sees another patient struggling to get out of a chair. She flashes back to being young and watching her mom struggle to get out of bed. Thirteen talks to Foreman about her concerns about being let into the clinical trials due to favoritism. Foreman assures her that she deserves to be in the study then conducts a test and tells her that her nerves have started degenerating.
Taub and Kutner monitor the woman who is using exercise equipment. Kutner notices that she looks faint, but she says she's fine, then falls over.
House goes back into his office and tells Cuddy that he will share his office with her. Taub and Kutner walk in and tell House that Emmy passed out again. House figures something is strangling her system and tells them to find the tumor and cut it out.
While looking over Emmy's x-rays, Kutner tells Taub that he set up a online practice. Taub asks if it's legal and Kutner says it is in most states. Taub asks if he can tell House and Kutner says he used House's name to start the site, so no. Taub says that if Kutner gives him 30% of the profits, he won't tell. They then look at images of Emmy's stomach and realize Emmy is lying about her all natural fitness routine.
House and the team look at old images of Emmy. Taub says that she had gastric bypass surgery, then hid all of her old photos so she could pretend she was always thin. Kutner suggests that Emmy could have had type II diabetes when she was younger. House agrees and says that she is likely suffering from nerve damage as a result of the diabetes. Kutner says that she may have sleep apnea, but House says no. House says that the bypass only made her skinny on the outside and she may still have her fat person's habits. Thirteen suggests malabsorption. Kutner says that she may have bacteria in her intestines. House says to test her poop to find out for sure.
While the doctors assess the patient, Cuddy talks loudly on her phone. House tries to stop her conversation by loudly talking about his balls. The doctors then leave. Wilson goes to Cuddy and asks her to admit she only wanted to take House's office to be near him. Cuddy tells him to be quiet or she will take his office.
Emmy and Taub argue about her methods for losing weight then trying to convince others to lose weight using an unsuccessful method. He gets a stool sample and takes it to Kutner. Kutner tells Taub about an email patient and Taub tells him that he is going to lose his license if he continues his online practice. House walks in and asks about the stool. They tell him that the test will take an hour. He says that fat floats so if he drops the poo in water, it should float. He drops the stool in water and it sinks.
Taub and Kutner check on Emmy and find that she is not in her bed. They find her running on a treadmill. They tell her that she is running on a broken ankle, but she says it doesn't hurt. They then point out that her leg is bleeding. They poke her leg and she says she can't feel it.
House and his team go to his office and find Cuddy who has dropped stinky chemicals on the floor. She then packs up and leaves. The doctors get in to the elevator. Foreman tells Thirteen to meet him downstairs for her treatments. She tells him she will be down in a little while. The doctors think that Emmy could be suffering from MS or Parkinson's. House says it could be a million other things and that they need to examine her brain.
Taub hooks Emmy up to the brain monitor. Emmy tells him that he was right that she is a hypocrite. She says that she is helping people live healthier, but if they find out she had surgery, they will stop listening to her. Taub asks Emmy to raise her arm, which she has trouble doing. Taub tells her that it looks like there could be something wrong with her muscles and they will need to run another test.
Taub and Kutner talk about Emmy's muscle weakness in the elevator. They notice a woman behind them and ask her where she is going. She pulls out her hair and says she needs to see House. Kutner realizes she is one of his online patients. The elevator doors open and Taub and Kutner see House on the bench. They decide to tell the woman House isn't in and take her downstairs.
